How to tell my lazy mil to help out around the house. *LAST UPDATE* I'M LEAVING.*

Some background info:

Mil has diabetes. (she doesn't make an effort to keep under control) she has bone and joint problems from a sedentary lifestyle, stones in her gallbladder.(she will have surgery in the coming months.) She babysits her daughter's 2 year old toddler and 2 month old son 4-5 days a week for about 5 to 6 hours a day.

Ok so my son is 5 months old and high needs. Mil lives with us because DH has been her provider for years. She always complains about her ailing health but blatantly disobeys nutrition and excersise tips to get better. Seriously, she sits on the couch ALL day. While I'm cleaning, cooking, putting groceries away and taking trash out one-handed with a cranky baby in the other. She's also not very big on cleanliness. She doesn't clean up after herself well, sometimes not at all. How can I tell her that she needs to help out without me looking like a total bitch? What can I say back if she tells me she's too sick and tired to help out?
